Harlem Hospital Center has been a bedrock of the Harlem community since 1887. Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Duties & ResponsibilitiesUnder the supervision of Ambulatory DON with varying degrees of latitude for independent initiative and judgment, coordinates and monitors the management of patient-centered quality care, ensuring optimal utilization of resources, service delivery, and compliance with external review requirements and applicable state and federal rules and regulations and nursing standards of care for better outcomes and improved patient experience.SUMMARY OF DUTIES AND RESPONSIBILITIES:
  • Works with interdisciplinary teams on patient experience initiatives and implementation of transformation developments across the continuum of care
  • Educates care teams on operations and designs programs to support patients in the practice
  • Responsible for chronic and preventative disease initiatives in accordance with evidence-based practice guidelines
  • Provides updates to the DON and collaborates on direction on implementation of strategic initiatives
  • Develops educational curriculums and provides training for ambulatory care transformation projects
  • Identifies barriers and works with the leadership teams to identify and implement solutions
  • Provides program information and activity to senior leadership team
  • Demonstrates knowledge of the patient population served and maintains a level of cultural competence in the patient management model
  • Provides direct coaching to supervisory staff (Nursing Supervisors, Head Nurses, and Assistant Head Nurses) and staff to improve individual performance
  • Attend departmental, interdepartmental and corporate nursing meetings as needed
  • Demonstrates knowledge of interpersonal/team building methods
  • Assists in the preparation, coordination, implementation, and evaluation of Training and Education Activities for care managers and Nursing Personnel
  • Maintains and updates clinical knowledge through continuing education
  • Performs patient assessment and provides care interventions and coordination across the health care continuum
  • Collect and analyze data for reporting and continuous performance improvement
  • Interview, orient, train, mentor, coach, supervise, schedule, delegate work, and evaluate staff performance
  • Participates in special projects and performs other duties as indicated
  • Under the direct supervision of the Director of nursing supports the vision and strategic direction for nursing.
  • Provides direct oversight of staff in assigned clinical department and/or area.
  • Assists in developing philosophy, goals, and objectives for department. Monitors metrics to assess and measure progress towards goals as appropriate, reports to DON in a timely manner.
  • Fosters a Patient-Centered culture that emphasizes patient safety and excellence in all aspects of care working collaboratively with the multidisciplinary care team to ensure the highest quality patient care.
  • Researches, recommends, and implements policies and procedures. Assists in the application of policies, programs and regulatory requirements to ensure compliance with all applicable standards.
  • Assists in the planning, implementation, and evaluation of clinical programs and services, assuring appropriate resources for the delivery of care.
  • Manage nursing budget and monitors variances against budget on an ongoing basis.
  • Assists in implementation of the patient care delivery model(s) utilizing the clinical and staffing standards. Makes recommendations and supports strategies for improving clinical processes and patient outcomes.
  • Serves as role model to staff promoting service excellence, quality outcomes and financial efficiency. Develops employee engagement strategies to promote employee satisfaction, retention, and talent acquisition. Promotes a culture of customer service aimed at creating an optimal patient experience and a collaborative work environment.
  • Assists in developing policies, objectives and standards of nursing service and education.
  • Participates in the direction and implementation of a quality assurance program for nursing. Provide oversight and facilitate the nursing QAPI program.
  • Manages employees by establishing annual performance goals, allocating resources, assessing annual performance.
  • Assists in the preparation of personnel, supply and equipment budgets.
  • Participates in management staff meetings and hospital meetings related to outpatient clinic activities.
Minimum Qualifications
  • Licensed and currently registered to practice as a Professional Nurse in New York State, plus (2) or (3) below.
  • A Master's Degree in Nursing Administration, Education or an equivalent field and a minimum of six years of satisfactory nursing experience, of which four years shall have been in a supervisory, teaching or administrative capacity; or,
  • A Baccalaureate Degree in Nursing and seven years of satisfactory nursing experience, five years of which shall have been in a supervisory or teaching capacity; or,
  • A satisfactory equivalent combination of education and experience. However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State.
