Job Description:

Summary

The Personal Care Worker (PCW) is responsible for providing essential personal care services to clients, assisting them with activities of daily living, and ensuring their comfort and safety. This role requires completion of agency training and a commitment to maintaining client confidentiality.

Qualifications

1. High school diploma or equivalent preferred.

2. Experience in personal care services in lieu of HS diploma.

3. Completion of agency and state required training and competencies.

4. Pass criminal background check.

5. Have state mandated immunizations.

6. CPR if state mandated.

7. Valid driver's license if required for the job.

8. English speaking, reading, and writing proficiency.

Job Responsibilities

1. Assist clients with daily tasks, personal care, mobility, and housekeeping as included in the service plan. This may include activities of daily living such as grooming, washing, bathing, dressing, toileting, feeding, cooking, cleaning, and laundry.

2. Assist clients with completing activities such as shopping and appointments outside the home.

3. Monitor medication adherence as prescribed for client.

4. Assist client with home program of exercises.

5. Completion of service notes and service provision for each visit. Documentation shall include date, time in and out, and confirmation of care provided. Additional information included is as per agency requirements.

6. Completion of electronic visit verification for clock-in and clock-out to each shift. Clock-out will include all tasks as per the Plan of Care performed for or refused by the client

7. Maintain confidentiality of patient information as per HIPAA guidelines.

8. Confer with supervisor as per client's progress, changes needed to service plan, and changes in patient condition. All incidents will be reported to supervisor promptly.

9. Maintain proper communication with client, family, supervisor

10. Follow all infection control precautions when providing services; maintain a home environment that ensures safety, health, and security of client.

11. Provide the client with support, companionship, conversation, and encouragement.

12. Always adhere to client rights.

13. Follow all emergency procedures when indicated.

14. Maintain a positive attitude and work atmosphere, communicate effectively, and develop a positive rapport with clients, families, coworkers, and supervisor.

15. Follow all agency policies and procedures.

16. Adherence to state and federal regulations, including mandatory reporting laws and continuing education requirements

17. Employee should be fit to drive, have clean driving record, and have valid insurance, if employee will drive as part of the job

18. Other duties as assigned by supervisor.

Physical Demands

The employee should be able to travel and sit/ stand for extended periods Employee should be able to lift, carry, transfer, and turn adult weight. Employee must be able to stoop and bend, and hear and effectively communicate. Employee should be fit to work without restrictions.
