Job DescriptionHealth Advocates Network is currently seeking a Audiologist to work at a facility in Perrysburg, New York. These are registry positions with our company.Pay Rate: $70 / hour*W2Shift details:
- 8: 30am and 3: 00pm Monday -Friday
Treatment Provider Qualifications Contractor must submit the Treatment Provider's rsumand reference list demonstrating they meet the qualifications listed below:1. The Treatment Provider must be a duly licensed Audiologist qualified to practice in the State of New York and registered to provide services as an authorized Hearing Aid Dispenser.2. Any contracted Audiologist providing Audiology Services is to be enrolled in Medicaid/Medicare. The basis of this requirement is for ability to process claims through Medicaid/Medicare.3. The Treatment Provider must be experienced in providing Audiology Services and have at least two years'experience in service to individuals with developmental disabilities.4. The Treatment Provider must not be on any state or federal list of individuals prohibited from submitting a Medicaid Claim.Services to be administered by Treatment Provider:1. Diagnostic Hearing testing based on referral.2. Impedance/Middle Ear testing.3. Hearing Aid evaluations, and possible procurement of hearing aids.4. Follow-up on routine basis on individuals who have hearing aids.5. Hearing aid analysis to detect breakdowns.6. Simple repairs of hearing aids.7. Counseling of individuals with hearing loss or processing problems.8. On-site training of support staff in routine maintenance and care of hearing aids.9. Treatment Provider will be required to complete Article 16 Clinic paperwork and billing to Medicaid, Medicare, and Third-Party insurance which will process for evaluation. Treatment Provider is expected to pursue through the National Provider ID number (NPI) and is expected to utilize clinic process to submit Medicaid billing for services. Treatment Provider will be required to process justification and necessary follow up for Medicaid reimbursement of hearing aids.10. Any on site work will require Sign-In and Sign-Out Sheets from the Treatment Provider. These will be signed off on by the Medical Director or their designee and will be submitted as part of the documentation for payment, as described under Section E, number 2. Accounting below under Invoices.11. Time spent in sessions and off-session time preparing notes, reports, consultations, correspondence, prescription writing, training, and other efforts are to be documented and denoted separately in a Service Log with times, dates, and location (or specify off site). These will be signed off on by the Medical Director or their designee This will be used for billing purposes and will be submitted with invoice, as described under Section E, number 2.
