Job Description:Primary Care Clinic at Tsehootsooi Medical Center.Title: Care Coordinator (case manager) nurse positions within our Primary Care clinic:Requirement: Three (3) year Case Management Experience.Shift: Weekdays Monday through Fridays from 8a to 5p, with One (1) hour lunch, no weekends or holidays.
