JOB SUMMARY
- Provides, or assists in providing, psychosocial rehabilitation services with residents of the facility. Services may include, but are not limited to, crisis management, individual and group counseling, assessment, treatment planning, MDS, discharge planning, case management, linkage, and referral.
QUALIFICATIONS
- Must possess a certificate or degree in Psychology, Social Work, or related field.
- Must have a minimum of one year experience working with the mentally ill (or similar) population.
- Must be able to read, write, and speak the English language in an understandable manner.
- Must be willing to work harmoniously with all personnel.
- Must possess the ability to deal tactfully with facility personnel, residents, family members, visitors, consultants, government agencies/personnel, and the general public.
- Must have patience, tact, a cheerful disposition, and enthusiasm.
- Must possess excellent communication abilities through listening, speaking, and writing skills.
- Must be able to maintain an appropriately professional manner in intense and/or crisis situations.
PHYSICAL REQUIREMENTS
- Must be able to move intermittently throughout the workday.
- Must be able to lift a minimum of 25 pounds.
- Must be free of physical limitation so as to be able to participate in physical C.P.I. (Crisis Prevention and Intervention) techniques as trained by certified trainer.
- Must be able to cope with the mental and emotional stress of the position.
- Is subject to exposure to infectious waste, diseases, conditions, etc., including exposure to HIV, AIDS, and Hepatitis B virus
ESSENTIAL RESPONSIBILITIES
- Prepare and provide clinically relevant interventions.
- Intervene with problematic and disruptive behaviors.
- Respond to crisis per facility policy and procedure.
- Follow and help maintain and enforce all facility rules and regulations.
- Complete all documentation as required by Federal, State, local and facility requirements.
- Assist in the over-all functioning of the behavioral health unit.
- Supervisory responsibility over the behavioral health unit.
- Attend and participate in all team meetings and supervisions as scheduled by Clinical Director of Behavioral Health Unit.
- Attend and participate in continuing education programs and in-services.
- Report all signs of resident abuse or neglect.
- Respect and enforce all resident rights.
- Work all shifts and start and end all shifts on time.
- Maintain personal and professional high standards of practice.
- Assist in conducting Comprehensive Assessments.
- Assist in development of Interim Treatment Plans, Comprehensive Treatment Plans and Quarterly Evaluations.
- Assist in the completion of the M.D.S. and R.A.P.S.
- Provide group and individual counseling sessions, as well as case management and discharge planning services.
- Assist in providing crisis management services, which may include the use of C.P.I. (Crisis Prevention and Intervention) techniques as trained by certified trainer.
- Assist in managing milieu and with mealtimes.
- Document assessments, interventions, contacts, and provision of services.
- Review relevant behavioral reports daily and follow-up as needed.
- Participate in and seek out clinical supervision.
- Assists the Activity Department with various activities.
- Assist other departments in managing resident behaviors.
- Attends all in-services and participate in Continuing Education.
- Perform other related duties as assigned.
