Join a purpose-driven community of champions.Position Purpose: Provide in office and community clinical assessment to determine services in an ethical, legal and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards. Participate as a team member in the delivery of behavioral health treatment services to patients and their families and facilitate the issues of recovery into their daily living situations.Job Requirements:
- Bachelor's in counseling, psychology, social work or another related field. Master's degree in licensable field strongly preferred.
- Must qualify as a MHP or higher per Illinois Department of Mental Health Rule 132
- Illinois Certified Addiction Counselor (CADC), or licensed in Illinois as a LSW, LPC, or higher, within 2 years of employment.
- Knowledge of DSM diagnoses and appropriate criteria for each diagnosis.
- Knowledge of accepted assessment techniques
- Ability to quickly establish rapport
- Computer proficiency required,
- Ability to navigate and complete documentation in the electronic healthcare record,
- Ability to accurately complete required documentation within the prescribed time frames
- Self-starter with ability to work independently,
- Ability to work as a team member.
- Valid driver's license and ability to meet Agency insurance requirements (designated positions),
- Must have own transportation (designated positions).
Essential Responsibilities:1. Work hours prescribed by the leadership and any additional hours deemed necessary for the responsibilities assigned.2. Provide centralized diagnostic assessments that include computerized social histories, mental status, presenting problem, diagnosis, and treatment needs.3. Determine appropriate level of care and treatment using centers utilization management criteria, develop corresponding ISP with clients, and coordinate needs with appropriate staff4. Identify the need for immediate crisis intervention services5. Maintain documentation per center quality assurance procedures and clinical performance standards6. Assess client needs and strengths and establish a comprehensive plan that addresses support and treatment needs necessary for less restrictive care and stabilization to occur safely7. Provide linkage, referrals and advocacy with community resources and community organizations8. Cooperate with work team/center/on accomplishment of strategic objectives and activities9. Ensure that the clinical treatment needs are met for consumers, which will enable them to work towards recovery.10. Educate community agencies regarding services offered by Rosecrance11. Serve as a member of the Access team and participate in all team meetings and activities.12. Exercise confidentiality in keeping with the Code of Conduct and with the framework of the law.13. Deliver exceptional customer service consistently to every customer.14. Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance15. Assume other related responsibilities as delegated and assignedSchedule: 8 hour shifts
- Monday - Thursday 10:30am-7:00 p.m., and then Friday 8:30-5:00p.m.
Work Location: Rosecrance Ware Center- Rockford, ILCompensation & Rewards
- Base Pay: Starting at $43,908.80 year (pay is based on education, experience, and credentials)
- Incentives:
- $.50/hr. with CADC certification
