PositionBehavioral Health Care Manager (LCSW or LMFT) Position Overview
Join our clients dedicated team under the guidance of the Behavioral Health and Care Management Leadership, where you will deliver exceptional care management services to our members. This role encompasses a broad spectrum of responsibilities, including member outreach and engagement, skilled therapeutic interactions to foster health-promoting behaviors, and ensuring seamless coordination of care. You will be instrumental in linking resources and collaborating with community professionals and organizations to guarantee top-quality member care. Your expertise will support smooth care transitions, ensuring members receive the right care at the right time. As a licensed clinician, you will provide clinical expertise, leadership, and oversight within the Behavioral Health and Care Management departments, promoting relationship-based care principles and enhancing the understanding of behavioral health's integral role in overall health among colleagues and the community.
Education & Experience
- Minimum of three (3) years of experience in Behavioral Health services, with a proven track record of successful teamwork and relationship-building with clients and coworkers.
- Masters degree from an accredited institution required.
- Active, unrestricted, and unencumbered LCSW or LMFT license in a Social Servicesrelated field required.
Key Qualifications
- Valid California Driver's license and valid automobile insurance.
- Familiarity with Managed Care and discharge planning is required.
- Proficient in evidence-based communication strategies such as Motivational Interviewing or similar empathy-based approaches.
- Understanding of and sensitivity to multi-cultural communities, with extensive knowledge of mental health and substance use conditions, including both acute and chronic management.
- Highly skilled in interpersonal communication with excellent relationship-building abilities.
- Resilient and adaptable to moderate changes and developments in new care models and care management practices.
- Proficient in computer software such as Microsoft Word, Excel, and PowerPoint.
- Able to engage members and providers effectively over the phone, building strong relationships in a phone-based environment.
- Capable of working as part of a team, executing duties, and making skillful decisions within ones scope of practice.

- Schedule: M-F, 8:00 am 5:00 pm
- Compensation: $43.87/hr
