Integrated care offers mental health providers new and exciting opportunities to become a new workforce of providing mental health delivery of the future understanding the close relationship between mental and physical well-being and enhancing access to care.Position Summary:The Behavioral Health Consultant (BHC) works as part of an integrated primary care team to provide brief, evidence-based interventions for patients with a range of behavioral health concerns. This position is embedded within the primary care setting and follows the Primary Care Behavioral Health (PCBH) model, collaborating closely with primary care providers (PCPs) to address both mental and physical health conditions. The BHC provides rapid, solution-focused consultations aimed at improving overall patient well-being, increasing access to care, and reducing stigma around behavioral health services.Key Responsibilities:Patient Consultation: Provide brief, targeted assessments and interventions for a wide range of behavioral health issues (e.g., depression, anxiety, stress management, lifestyle changes) within the primary care setting.Integrated Care Collaboration: Work closely with primary care providers (PCPs), sharing findings and recommendations to assist in whole-patient care. Provide ongoing consultation to medical staff on behavioral health matters.Evidence-Based Interventions: Utilize brief, evidence-based interventions such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and problem-solving therapy, ensuring services are tailored to the fast-paced primary care environment.Care Coordination: Help to coordinate care with community mental health services, specialists, and external resources when patients require longer-term or more intensive care.Behavioral Health Screenings: Administer and interpret brief behavioral health screenings (e.g., PHQ-9, GAD-7), providing feedback to both patients and the primary care team.Documentation: Maintain thorough and timely documentation of patient encounters, assessments, and treatment plans in the electronic medical record (EMR).Psychoeducation: Provide patients and families with education on behavioral health issues, coping strategies, and self-management tools to improve overall well-being.Crisis Intervention: Offer brief crisis interventions as needed and collaborate with the care team to develop safety plans and appropriate referrals.Health Behavior Change: Support patients in managing chronic health conditions by promoting positive behavioral changes, including diet, exercise, medication adherence, smoking cessation, and stress reduction.Group Sessions & Workshops: Facilitate group sessions and educational workshops for patients dealing with common concerns such as stress management, sleep problems, and managing chronic illnesses.Qualifications:Education & Experience:Master's in Counseling or Social Work in a program leading to licensure or a Doctoral degree in Clinical Psychology.Recent Master's level graduates considered (MHCs and LMSWs).Current NYS licensure or eligibility for licensure (LCSW, LMHC, LMFT, PsyD, PHD).Minimum of 2 years of experience in behavioral health, preferably in a healthcare setting or integrated care model preferred.Experience with evidence-based treatments including but not limited to CBT, Motivational Interviewing (MI), and mindfulness- based interventions. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills.
