Progressive Community Health Centers is seeking an experienced professional to serve as a Behavioral Health Navigator at our Lisbon Avenue Health Centers.Does not require working late nights, no holidays, and no weekends!Who are we?Founded in 1999, Progressive Community Health Centers is an independent non-profit organization that operates four clinic locations in Milwaukee, Wisconsin. More than 100 employees support our agency's mission to improve the health and quality of life of the community by providing culturally competent services that address identified needs. We provide high-quality, comprehensive care to roughly 16,000 patients annually majority of who are low-income. No one is ever turned away from our clinics based on insurance status or ability to pay. We value a patient-centered philosophy and provide a range of services that include family medicine, internal medicine, women's health, urgent care, dental, radiology and integrated behavioral health. We also offer supportive services such as case management, health education, and benefits enrollment.What will you do?The Behavioral Health Navigator involves implementing care plans designed by social workers to address patients' identified needs, resources, and goals. Key responsibilities include educating patients and their families about available options, prioritizing cases based on urgency, and effectively utilizing community resources. The position promotes patient self-management and independence, assisting with essential services like transportation and housing.Additionally, the role requires maintaining accurate documentation, participating in departmental meetings, and fostering collaborative teamwork while adhering to organizational policies and ethical standards. The candidate must possess strong communication skills, cultural competency, and the ability to work independently and as part of a team. Knowledge of federal and state regulations, particularly regarding patient confidentiality and HIPAA, is essential.Schedule: Monday - Friday, 8 a.m. - 5 p.m.Responsibilities
  • Implements the plan developed by the Social Worker that addresses the patient's identified needs, issues, resources, and care goals; determining the choices available to individual patients; and educating the patient/family on available options; prioritizing patients according to intensity, need, and required follow-up.
  • Identifies and effectively utilizes community resources to meet the needs of patients/families.
  • Promotes patient self-management and empowers patients and families to achieve maximum wellness and independence. Assists patients with needed items, including transportation, scheduling, housing, food sources, etc.
  • Maintains accurate and timely documentation.
  • Participates in departmental and organizational committees as applicable.
  • Promotes collaborative teamwork.
  • Maintains appropriate professional boundaries.
  • Contributes to developing a social work program and plans for future expansion.
  • Adheres to organizational policies and procedures. Provides input into work processes and policy revisions.
  • Maintains a working knowledge of and adheres to applicable federal/state regulations, including, but not limited to, laws related to patient confidentiality, release of information, and HIPAA.
  • Uses safe work practices. Promptly reports workplace and patient safety issues to supervisor.
  • Interacts in a manner that is professional, respectful, positive, helpful, and promotes trust.
  • Maintains professional growth and development.
  • Acts in a manner consistent with the Code of Conduct.
  • Other duties as assigned.
  • EDUCATION AND EXPERIENCE:Required Qualifications:
  • A bachelor's degree in social work, human services, psychology, public health, criminal justice, sociology, anthropology, or related field.
  • Related Work Experience
  • Experience preferred within Milwaukee County community resource settings.
