Job Type Full-time Description BEHAVIORAL HEALTH PROVIDER (LSW/LISW/LPC/LPCC/IMFT)SUMMARY: Provide psychosocial assessment, consultation and brief intervention for psychological/psychiatric problems and/or disorders including chemical dependency SUPERVISOR: Associate Medical Director DUTIES AND RESPONSIBILITIES:
- Provide effective treatment planning and assist clients with successfully achieving goals.
- Assist patients and their families with adjusting to episodes of acute and chronic illness and coping with lifestyle changes to promote positive outcomes.
- Evaluate crisis situations and apply appropriate interventions.
- Actively participate in meetings that support AxessPointe Community Health Center, Inc.(API) Integrated Health Care model to provide comprehensive care for clients.
- Assist in the detection of at risk patients and development of plans to prevent further psychological or physical deterioration.
- Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
- Provide comprehensive counseling of patients enrolled in medication-assisted treatment (MAT) program via collaborative and integrated care with the primary care team in successful management and treatment of chemical dependency.
- Monitor the site's behavioral health program, identifying problems related to patient service and making recommendations for improvement.
- Facilitate outreach and involvement with community-based programs and agencies.
- Provides consultation to other health care professionals when coordination with significant or intensive community resources is necessary to achieve desired treatment goals.

Requirements MINIMUM QUALIFICATIONS:
- Master's Degree in Behavioral Health, Social Work, Counseling, or other related field with the capacity to bill for encounters desired.
- Working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
- Ability to work through brief patient contacts as well as the ability to make quick and accurate clinical assessments of mental and behavioral conditions.
- Should be comfortable with the pace of primary care, working with an interdisciplinary team, and have strong communication skills.
- Good knowledge of psycho-pharmacology.
- Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions.
- Multilingual, multicultural and culturally diverse applicants encouraged.
- Ability to effectively work with diverse clientele, Agency staff, and outside contacts.
LICENSES OR CERTIFICATIONS REQUIRED:
- Licensed in the State of Ohio as a Licensed Independent Social Worker (LISW)/Licensed Social Worker (LSW) required. Or
- Independent Marriage and Family Therapist Or
- Licensed Professional Clinical Counselor (LPCC)/Licensed Professional Counselor (LPC)
