DescriptionPay $56,160 - $72,800 annuallyUpstate Caring Partners' Certified Community Behavioral Health Clinic - Community Health and Behavioral Services is seeking compassionate and skilled Behavioral Health Provider's to join our Integrated Care Team. In this role, you will be instrumentalin delivering comprehensive, integrated mental health services to clients with co-occurring mental health and substance use needs. Our clinic emphasizes a whole-person orientation to behavioral health, addressing the physical, emotional, and social aspects of well-being across the lifespan.The Behavioral Health Provider will work within a multidisciplinary team to ensure that clients receive coordinated, holistic care that supports their recovery and enhances their quality of life at every stage of development. The position offers a tiered system (I, II, III) based on skillset and licensure status, providing opportunities for professional growth and development within the organization.Core Responsibilities+ Conduct thorough assessments of clients with co-occurring mental health and substance use disorders, utilizing evidence-based assessment tools.+ Develop and implement individualized treatment plans that address both mental health and substance use needs, incorporating input from the client and the Integrated Care Team.+ Monitor client progress, adjusting treatment plans as necessary to achieve optimal outcomes.+ Provide individual, group, and family therapy sessions using evidence-based practices, such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and Integrated Dual Diagnosis Treatment (IDDT).+ Deliver substance use counseling, relapse prevention strategies, and psychoeducation tailored to clients' needs.+ Collaborate with other team members to ensure coordinated care that addresses all aspects of clients' mental health and substance use challenges.+ Work closely with case managers, medical providers, and peer support specialists to ensure clients receive comprehensive care and support.+ Coordinate referrals to external services, such as inpatient treatment, detox programs, or community support groups, as needed.+ Maintain regular communication with clients and their families to support ongoing engagement and adherence to treatment plans.+ Maintain accurate and timely clinical documentation, including assessments, treatment plans, progress notes, and discharge summaries, in compliance with clinic policies and regulatory standards.+ Ensure all services are delivered within the provider's clinical scope of practice, aligned with relevant licensure and professional standards.+ Participate in regular supervision, case reviews, and team meetings to ensure high-quality care and professional development.+ Actively participate in multidisciplinary team meetings, contributing to the development of integrated care plans that address the holistic needs of clients.+ Collaborate with medical providers to manage clients' psychopharmacological needs, ensuring a coordinated approach to treatment.+ Engage in community outreach and collaboration with external partners to enhance service delivery and client outcomes.+ Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional conferences.+ Accomplishes organization goals by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to the physical health department.Professional Development and Tiered Advancement+ Tier I (Unlicensed/Limited Permit Professionals):+ For professionals who are unlicensed or hold a limited permit (e.g., Master's degree in Social Work, Counseling, Marriage and Family Therapy, or a related field, working toward licensure).+ Providers in Tier I will operate under close supervision, focusing on building foundational skills in treating co-occurring disorders while working toward full licensure.+ Tier II (Entry-Level Licensed Professionals):+ For professionals with initial licensure (e.g., LMSW, LMHC with no Diagnostic Privileges, LIC, LCAT, or equivalent) who are developing their clinical practice in co-occurring disorder treatment.+ Providers in Tier II will receive ongoing supervision and training, gradually increasing their independent practice responsibilities.+ Tier III (Intermediate Licensed Professionals):+ For professionals with advanced licensure (e.g., LCSW, LPC, LMFT, LMHC with full Diagnostic Privileges or equivalent) and demonstrated competency in treating co-occurring disorders.+ Providers in Tier III function more independently and contribute to program development, as well as mentorship of Tier I and Tier II staff.Qualifications+ A minimum of a master's degree in social work, counseling, marriage and family therapy, or a related field is required.+ Relevant licensure or limited permit status is required, with the tiered system determining scope of practice.+ Prior experience in a behavioral health or community health setting is preferred.+ Valid New York State license and current registration+ Must have a valid NYS Driver's License.+ The Behavioral Health Provider may be required to work flexible hours, including occasional evenings and weekends, to meet the needs of the clinic and the clients.BenefitsOur work environment provides the opportunity to grow and learn in a fast-paced atmosphere.
