Valley-Wide is an approved site with the Colorado Health Service Corps and National Health Service Corps loan repayment programs. Providers are eligible to apply and may be awarded up to $90,000.00 for providing care in our rural communities.Valley-Wide Health Systems, Inc. As a non-profit, Federally Qualified Health Center (FQHC), we provide premier primary and preventive care you can trust at our 34 service sites strategically located throughout multiple rural counties.We are looking for a Behavioral Health Provider to work at our Sierra Blanca Medical Center in Alamosa, CO. The Behavioral Health Provider will work as part of an integrative, primary healthcare network, using a Team-Based Care (TBC) approach to focus on holistic care for patients, across all ages with the goal to provide preventive, therapeutical and educational services. Fostering a culture of patient and family-centered care, service excellence and safety while enhancing patient, employee and community relations.Job Responsibilities:* Provide clinical assessment, problem identification, need for medical necessity (DSM 5 criteria/justification), evidence based treatment modality, crisis intervention, individual/family/group therapy or other activities, case management, advocacy and outreach/prevention and interventions* Collaborates with the care team, following the Primary Care Behavioral Health (PCBH) model, developing and implementing evidence-based treatment plans and interventions through consultation, brief assessment, and psychoeducation.* Document patient interactions ensuring clinical charts within EMR system (NextGen) are kept up to date.
