: Job Details :

Practices in the home and community to provide behavioral health treatment. Conducts psychosocial assessments and living conditions of patients and social determinants of health and develops treatment recommendations. Provides advocacy, psychoeducation, cognitive/behavioral interventions, monitoring of mental status, emotional support, brief psychotherapy, and/or family-based interventions. Collaborates with interdisciplinary team members to achieve successful patient outcomes.
