Your RoleResponsibilitiesAs a Registered Nurse - Behavioral Health, you will be an integral part of a team dedicated to providing compassionate and comprehensive care to patients dealing with behavioral health challenges. Your responsibilities will include:
- Assessing and coordinating care: Evaluate patients' physical, psychosocial, and environmental needs through clinical records, reports, and on-site evaluations.
- Developing and implementing care plans: Take primary responsibility for creating, executing, and updating treatment plans for patients based on their needs.
- Collaborating with teams: Communicate and collaborate with physicians, clinical supervisors, family members, and other caregivers to ensure patient care is holistic and effective.
- Providing education and support: Educate patients, families, and staff on behavioral health, symptom management, and care philosophies.
- Supervising and mentoring: Guide behavioral health aides and LPNs in delivering care, ensuring quality standards are met.
- Caring for patients at end-of-life: Provide support during critical moments, including death and bereavement assessments.
What We're Looking ForWe are looking for compassionate Registered Nurses who are committed to improving lives in the comfort of patients' homes. Ideal candidates will have:
- A current, unrestricted state RN license
- 1+ years of experience in a clinical care setting
- Ability and willingness to travel up to 50% within your coverage area
- Reliable vehicle, valid driver's license, and current auto insurance
- Preferred: Associate's or Bachelor's Degree in Nursing
- Previous Home Health or Hospice experience a plus
- Familiarity with Homecare HomeBase is preferred
