Milestone Centers has earned certification from the Sanctuary Institute for providing trauma-informed care.Responsibilities:POSITION SUMMARY: As a Behavioral Health Therapist in the social rehabilitation program, the primary focus is to empower individuals to achieve their full potential. Milestone offers services and support designed to improve overall health outcomes by targeting key areas of Holistic Health that impact well-being beyond traditional healthcare. Key areas of program focus include:Supportive person-centered activities that promote safe and sustainable independent livingOpportunities to strengthen community engagement and social support for those servedEducational activities and health literacy promotionEnsure access to healthy food and nutritionImprove economic stability through exploration of employment related activitiesThis multifaceted role involves providing support to those served in the social rehabilitation program through conducting comprehensive assessments and developing personalized plans to guide each person's unique journey towards independence, recovery, and reintegration. A key aspect of responsibility is facilitating engaging group therapy sessions, both within the facility and in the local community. These sessions are designed to cultivate a sense of shared experience and mutual growth among individuals with diverse abilities, including those navigating physical or intellectual differences, those experiencing behavioral health journeys, and members of the vibrant Deaf community.In addition, depending on the person-centered needs of the person served, one on one support may need to occur to support the person. By providing expert support and tailored interventions, you will play a vital part in helping individuals overcome challenges and develop essential life skills. This role is instrumental in creating a supportive environment that fosters healing, personal growth, and lasting positive change for those served.ESSENTIAL FUNCTIONS:Collaborate with people to establish realistic, measurable, and achievable treatment goalsAssessing people's needs, interests, and abilities to develop personalized activity plansDevelop individualized treatment goals for each client based on their unique needs, strengths, and desired outcomesFormulate comprehensive treatment plans that outline the specific interventions, strategies, and resources necessary to help people served reach their goalRegularly review and update treatment goals and plans in response to client progress, changing needs, or new circumstancesProvides people supported activity opportunities to foster skills in facility or community settingsFacilitate dynamic and engaging activities or discussions that promote personal growth, self-awareness, and skill development, honoring the diverse perspectives and experiences within the group which meets individualized objectivesImplement applicable behavioral support plans and utilize positive strategies to support the person receiving servicesImplement individualized crisis plan, if applicable, as per regulations and guidelinesCollaborate with a multidisciplinary team to ensure comprehensive care and support for each people servedConsults with Supervisor and other external service providers and linkage with other levels of care within the Agency if a need is identifiedStay current with the latest therapeutic approaches and best practices in social rehabilitationResponsible for completing accurate and professional documentation in a timely manner. This documentation encompasses a range of materials, including applicable assessments, progress notes, census and billing records, and other related programmatic documentation.
