Job Responsibilities & Qualifications The Behavioral Health Worker will provide case management services in coordination with the Behavioral Health Consultant. This position is mostly case management related work and requires extensive knowledge about community resources. The ideal candidate has a Bachelor's degree in a mental health discipline or related field and at least 1 year of experience in case management or other, similar work. Primary Tasks and Responsibilities:
Provide referrals to behavioral health providers and coordinate initiation of services with patients based on individual needs. Coordinate access to other ancillary services as needed. Communicate and collaborate with the Behavioral Health Consultant and medical team about patient needs and access to care. Crisis intervention as needed. Participate in daily staff huddles and weekly case conferences to collaborate with other professionals and across clinics. Document interactions in EMR. Other duties as requested or assigned. Qualifications:
- Bachelor's degree in behavioral health or related field, e.g., psychology, social work, counseling, etc.
- Minimum of 1 year of experience providing case management services or similar work.
- Excellent interpersonal skills and ability to work collaboratively with medical providers, support staff, and other professionals across multiple disciplines.
- Experience with diverse populations
