Description:GENERAL DESCRIPTION:Carolina Health Centers, Inc. is looking for a Behavioral Health Consultant (BHC) to grow their Behavioral Health Department and join their primary care team. Carolina Health Centers, Inc. is a decorated patient-centered medical home that serves children and families throughout the Lakelands area. The BHC will be embedded into Carolina Health Centers, Inc.'s LC4 location and serve as an integral member of a multidisciplinary care team. The BHC will serve patients across the lifespan by delivering targeted behavioral health services.The BHC will:· Conduct screenings, assessments, and brief interventions to our patient population· Assist the primary care provider in recognizing, treating, and managing behavioral health concerns· Utilize evidence-based interventions· Provide same-day warm handoffs as well as real-time consultation to medical providers· Conduct follow up appointments and/or ongoing therapy with patients, as appropriate· Clarify mental health diagnoses when needed· Collaborate with other medical professionals in a fast-paced work environment· Connect patients with additional resources, as needed· Work with a variety of mental health concerns with individuals of various agesRequirements:All employees of Carolina Health Centers, Inc. are expected to perform the duties of their job and behave in a manner consistent with the Corporate Philosophy which supports the values of: honesty, integrity, openness, the pursuit of individual and collective excellence, and unwavering mutual respect and appreciation.In addition, this position requires:Master's degree and professional licensure in the state of South Carolina (LMSW, LISW-CP, LPC-A, LPC, LPCS, LMFT-A, or LMFT)Experience and Skills:· Ability to effectively collaborate with an interdisciplinary care team· Familiarity with brief, structured intervention techniques (CBT, Motivational Interviewing, etc.)· Ability to respond effectively to clinic needs and to a fluid schedule· Ability to provide patient updates in a clear, concise manner· Create treatment plans that support overall health goals· Coordinate patient care with community partners· Proficiency with electronic medical recordsPay: $58,618.66 - $87,927.99 per year
