Your Impact as a Behavioral Health RN Case Manager:
* Patient Care Management: Manage a caseload of behavioral health patients, assessing their health, psychosocial needs, and ensuring they receive the care they deserve.
* Plan Development: Collaborate with an interdisciplinary team to craft personalized care plans, adapting as needed to meet changing patient conditions.
* Communication & Collaboration: Partner with physicians, clinical supervisors, and healthcare team members to ensure top-quality care and timely interventions.
* Education & Support: Empower patients, families, and facility staff through education and support, enhancing the care experience.
* Leadership: Supervise and mentor behavioral health aides and LPNs, ensuring consistent, high-quality care delivery.
What You'll Bring:
* Current RN License (unrestricted in the state)
* 1+ year of clinical experience (Behavioral Health, Home Health, or Hospice experience preferred)
* Reliable Transportation: Must have a dependable vehicle, valid driver's license, and current auto insurance to visit patients.
* 50% Travel Required within the service region.
* Educational Background: Associate's or Bachelor's Degree in Nursing (preferred).
* Experience: In Home Health and or Hospice is preferred
