| Poughkeepsie, NY |Poughkeepsie, NY| Full-time |Full-time| $60,000 - $62,000 a year |$60,000 - $62,000 a year| As a member of the Hudson Valley Hospice Bereavement team the primary focus is to provide bereavement counseling and emotional support to patients, families and staff as deemed necessary to help them understand issues related to the dying process, grief, and loss. As an LMSW, you will be responsible for a case load of clients to assist with bereaved services. Responsibilities Include: Follows appropriate standards of practice established by the National Association of Social Workers and the National Hospice and Palliative Care Organization guidelines for Bereavement. Attends and/or participates effectively in annual Ceremony of Remembrance. Completes bereavement initial assessment and formulates plan of care according to presenting needs of client. Provides grief counseling based on companioning method. Individualized interventions include but are not limited to active listening, validation, reframing, providing grief education, and developing coping strategies in a supportive environment. Performs ongoing assessment to facilitate client-centered approach to counseling. Provides crisis intervention as needed, including following screening and safety protocols for those expressing suicidal ideation. Reports all such cases to supervisor. Effective screening of bereaved individuals to encourage appropriate group participation. Desired Qualities: Ability to use time management effectively to appropriately plan for all responsibilities, including team meetings, individual and group sessions, and documentation. Demonstrates knowledge of community resources and educates clients as needed. Ability to recognize client strengths and weaknesses. Ability to effectively communicate verbally (in person and by phone) and in writing. Ability to prioritize and manage multiple tasks efficiently. Excellent attention to detail. Desired Education and Experience: Master's degree in social work and valid New York State Social Work License, or LMSW eligible Minimum of two years of social work experience with one-year experience in grief counseling, hospice and/or community health preferred. Knowledge of unique issues related to death, dying and grief preferred. Valid New York State driver's license with reliable transportation. Hours: Monday, Wednesday, Thursday and Friday (8:30am-4:30pm) and Tuesdays (10:30am-6:30pm) to accommodate for group session FLSA: Exempt Pay: $60,000-62,000 per year Covid-19 Precautions: Hudson Valley Hospice requires the Covid-19 vaccine and one booster.
