Position Overview:
The Behavioral Health Integration Program is part of NYS Article 28 program. It aims to provide short-term treatment to a diverse patient population, behavioral health consultation for PCPs and other medical staff, and patient connections to outside services not provided by Ryan. A vital part of our treatment is assessment and treatment planning and includes the implementation of universal mental health screenings for depression, anxiety and substance use and abuse.
The Behavioral Health Integration Clinician (LCSW/LMSW) is responsible for providing short term mental health services to the patients of Ryan Health. These services include outreach, clinical screenings, psychosocial assessments, and psychodynamic psychotherapy utilizing evidenced based practices. The Clinician functions as a team member of an interdisciplinary staff to ensure integration of care for Ryan patients, and to maximize the efficacy of screening, referral, and follow up communication between primary care and mental health providers. The Clinician will work with community agencies to assure coordinated care for their patients. This work is carried out in support of the mission and goals of Ryan Health.
Essential Functions:
* Maintain an assigned caseload of patients with diverse mental health needs
* Provide short term supportive and insight therapy using a variety of treatment modalities including individual, group and family therapy.
* Provide support to patients in crisis.
* Administer mental health screenings, clinical assessments, recommendations, treatment planning, internal and external referrals, and triage as needed.
* Develop linkages with outside agencies including mental health and substance abuse providers, local social service agencies, and other resources to meet the psycho-social needs of our patients.
* Participate in staff meetings and case conferences with the BHI team and mental health teams throughout the network.
* Maintain professional competence by attendance at professional conferences, trainings, in-service seminars and other meetings as assigned.
* Participate in weekly individual supervision.
* Adhere to all agency and departmental policies and procedures.
* Must demonstrate a professional, courteous and respectful attitude in dealing with patients, immediate supervisor, and other employees.

* Immediately report any problems or unusual occurrences to supervisor/s.
* Perform other duties/special assignments as assigned.
Qualifications
Minimum Experience and Skills Required:
* Two years' experience in a health/behavioral health setting as a social worker
* Must be knowledgeable and trained in some evidenced based practices.
* Able to practice using multi-modalities, individual, group and/or relationship therapies.
* Demonstrated track record of organization, focus and ability to work in a fast-paced clinic.
* Working experience with electronic medical records software.
* Able to use a computer for word processing and scheduling appointments in a calendar.
* Experience treating individuals with mental illness and working with diverse patient populations both children and adults.
* Able to meet the demands of a busy clinic practice in both serving patients well and documenting the work done in a timely manner.
* Demonstrate cultural competency
Education, Experience and/or Skills Required:
* Master's Degree from an accredited School of Social Work
* Licensed Master SW or Licensed Clinical SW
* Licensed to practice in the State of New York
Education, Experience and/or Skills Preferred:
* eClinicalWorks
* Bilingual: English/Spanish.
