Job DetailsJob LocationBronx, NYPosition TypeFull TimeSalary Range$22.00 - $22.00 HourlyDescriptionPOSITION: Bilingual Case Management Support Specialist - Unaccompanied Minors ProgramREPORTS TO: Quality Improvement Specialist/Lead Case Aide CLASSIFICATION: Full Time EXEMPT/NON-EXEMPT: Non-Exempt (Hourly)RATE: $22 an hourGENERAL DESCRIPTION: The Lutheran Safe Haven Shelter and Transitional Foster Care Program is a 24-hour program that serves unaccompanied children (UCs) in immigration custody who are awaiting release to their sponsors. The Case Management Case Aide will provide administrative support to the Case Managers in the evening and weekend hours.MAJOR DUTIES AND RESPONSIBILITIES INCLUDE:
  • Complete Intake Assessments and Screen for human trafficking concerns.
  • Document in detail the provision of services in each UC's case file.
  • Assist potential sponsors with completion of family reunification packets and refer them to fingerprinting.
  • Assist in the supervision of UCs when needed to maintain proper ratio.
  • Review, scan and file UC logs and other documentation as needed.
  • Create UC charts and ensure that program documents are properly filed for each UC, with case manager coordination.
  • Maintaining and organizing charts accordingly.
  • Complete chart audits and portal checks to ensure all documentation is audit ready.
  • Document and report all major and minor incidents in accordance with agency policies and procedures.
  • Ensure all weekly and monthly reports and spreadsheets are completed on time.
    • Ensure compliance with local, state, and federal rules and regulations, including but not limited to HIPAA.
  • Required to work evening and weekend hours on an ongoing basis.
  • Other duties as assigned.
QualificationsEDUCATION/EXPERIENCE: associate and experience working with immigrant youth or child welfare services preferred.SKILLS & ABILITIES:
  • Good communication and writing skills in English and Spanish.
  • Ability to work collaboratively with others.
  • Superior organizational skills, resourceful, and able to manage time effectively.
  • Experience and facility with Microsoft Applications
