SCHEDULE Monday - Friday, 9:00AM - 5:00PM (with flexibility for occasional evenings or weekend events). SUMMARY Family Service League is seeking a full-time Bilingual Case Manager for the Healthy Families Suffolk program. The Bilingual Case Manager will be responsible for reviewing referrals of prospective participants to determine program eligibility, and conducting interviews and completing assessments with expectant parents or parents who have a child less than three months of age. The Bilingual Case Manager will provide home visits to families located in the program's catchment area: Amityville, Bay Shore, Brentwood, Central Islip, Copiague, Huntington, Huntington Station, and Wyandanch. RESPONSIBILITIES
  • The Case Manager will conduct home visits to assess family needs and provide case management services, when necessary, while promoting self-sufficiency.
  • Conduct assessment in the participant s home through a one-to-one conversational weave process with all eligible families during the prenatal or postpartum period.
  • Thoroughly document assessments and home visits in narrative form.
  • Meet with supervisor weekly to review assessment information, appropriateness of referrals for services, case assignment. Final determination to be made by supervisor based on availability of staff and need for services.
  • The Case Manager will maintain necessary documentation in accordance with agency and funding source requirements.
  • Submit documentation on time to the Supervisor.
  • Coordinate with other community services.
  • Attend and participate in all scheduled Healthy Families conferences, outreach, seminars, in-service training and parent events.
  • Serve as an advocate for participants and potential participants as required by linking and/or referring families to appropriate community resources.
  • Plan and provide short term direct assistance if required with the supervisor s approval.
  • Maintain tracking forms, required statistical information, data forms, and other paperwork as required, in a detailed and timely manner.
  • Collaborate with community partners to maintain an effective referral base.
  • Outreach in the community, as needed, to maintain an effective referral base.
  • The Case Manager will maintain flexibility in scheduling (able to work limited nights and weekends).
  • Participate in team activities and agency Continuous Quality Improvement activities.
  • Establish trusting relationships with families and work effectively with mothers, fathers and extended family.
  • All other duties as assigned.
QUALIFICATIONS High school diploma or equivalent required. NYS-AIMH Infant Mental Health Endorsement is a plus. Experience in working with or providing services to children and families. An ability to establish trusting relationships. Acceptance of individual differences. Experience and humility to work with culturally diverse families. Knowledge of infant and child development. Willing to engage in building reflective capacity (e.g., capacity for introspection, communicating awareness of self in relation to others, recognizing value of supervision, etc.). Excellent verbal and written communication skills required. Proficient computer skills, including Microsoft office, required. Valid and clean NYS Driver s License required. Bilingual in Spanish required. PHYSICAL REQUIREMENTS Office, community, and client home based work with frequent sitting, standing, writing and computer usage. Must be able to climb stairs and move through a facility and a home. Moderate reaching, bending, stooping kneeling or crouching. The position includes interaction with children that could include picking up a child as heavy as 40 pounds. The nature of the position involves a fast-paced working environment with multitasking of many different duties and tasks. General daily priorities may change at a moment s notice and the position requires quick response time and flexibility.
