Job Description:

Overview:

The C.N.A will interact directly with one or more consumers, providing health care services, perform day-to-day tasks and play a vital role to consumers within the home while maintaining a professional and healthy environment.

C.N.A may work under the supervision of PCT-Advanced and/or the House Manager / Case Manager. Also, the C.N.A position may require weekend shifts. The specific job duties for the C.N.A are designed around the needs of each individual consumer and may be modified as the needs of the consumer change.

Key Responsibilities:

Provide instruction and assistance in daily activities and routines such as:

* Assist with Daily Living Activities (ADLs):

o Personal Hygiene - bathing/showering, grooming, nail care and oral care

o Dressing - assisting with weather appropriate clothing and physically dress/undress

o Eating - the ability to assist with oral feeding, G-tubes, or J-tubes

o Toileting - the ability to assist with transfers on/off of the toilet, commode, or changing incontinence products, ostomy, and catheter care

o Transferring/Mobility - assisting from seated to standing position, getting in/out of bed, and the ability to use equipment such as a hoyer lift or EZ-stand lift

* Assist with Instrumental Activities of Daily Living (IADLs):

o Transportation - If applicable driving consumers, arranging rides, or assisting to set up public transportation

o Meal preparation - meal planning, cooking, cleaning up, storage, and the ability to safely use kitchen equipment and utensils

o Shopping - ability to make appropriate food and clothing purchase decisions

o Housework - laundry, washing dishes, dusting, vacuuming, and maintain a hygienic place of residence

o Administration of Medications-orally, g-tube, topically, SQ injections, eye, ear, rectal, nasal, and inhalers

* Obtaining vital signs: blood pressure, pulse, respiration's, temperature, and oxygen saturation (pulse ox)

* Examining/Monitoring consumers to see if medical care is needed, including skin integrity, physical or mental changes

* Notating observations of consumer behavioral, including complaints or physical symptoms

* Comply with HIPAA Privacy Act by protecting confidential consumer information

* Able to maintain current updates on C.N.A training's and employer policies and procedures

* Obtain a wide range of consumer information physicians, nurses, house managers /case managers, care plans and suggested activities

* Other duties as assigned

Preferred Knowledge, Skills, and Abilities:

* High school diploma or equivalent

* Wisconsin Certified Nursing Assistance Certification

* Must be able to work independently with minimal supervision

* Functional reading and writing skills

* Ability to work flexible hours and unconventional schedules, including evenings, weekends, overnights, and holidays

* Able to pass Care Giver background requirements per state law

* Regular attendance and punctuality (may need to cover shift until relief worker arrives)
