  • The hours are 10:45 PM to 7:15 AM
  • 32 hours per pay period
  • Includes every other weekend and holiday
  • Supporting ourNelson Gables Facility
Scope:
  • Provide leadership and direction to team during assigned shifts.
  • Train staff as assigned.
  • Provide care to residents according to care plans.
  • Proper documentation according to regulations.
  • Partner with meals services, Life Enrichment and Wellness for resident well-being.
Key Responsibilities:
  • Resident Care and Services - Provide residents individualized, direct care with activities of daily living which may include some or all of the following: showering, bathing, grooming, dressing, oral care, toileting, transferring, fingernail cares and foot cares. Will assist with meals, restorative care and life skills based upon resident preferences, regulations and established best practices. Maintain all required documentation in accordance with regulations and established clinical best practices. Performs medication administration and other delegated nursing procedures. Take and record temperature, weights, blood pressure, pulse and respiration as instructed and communicate any changes or concerns to the nurse. Observe client and communicate changes in physical and emotional condition. Support residents in self-care and provide care as necessary for those unable to care for their own needs. Observe residents closely, identifying changing needs and conditions at first indication, and reports immediately to nursing as appropriate. Participate actively in the resident's care planning, serving as the residents' advocate in provision of care and honoring their preferences. Consistently work with all team members, led by resident and family preference to meet the needs and desires of all residents. Demonstrates flexibility in adjusting priorities in response to resident needs.
  • Leadership and Mentoring - The mentor will provide hands on support for the team, ranging from new hires to seasoned staff within the Housing division of Knute Nelson. Examples of support and tasks include overview and education on organizational structure, mission and values, being a positive role model, remaining accessible, committed, and engaged to goals and outcomes. Mentors will follow our campus Team Charter norms and customer service expectations and lead the team by example. Mentors will present with kindness and positivity and be warm and welcoming to others, creating a positive team environment for all. Will engage in open, constructive conversation and problem solving. Mentors will ensure quality care is provided to all residents and all staff are equipped with appropriate guidance and feedback. Will be expected to attend monthly mentor meetings and formal education/training.
  • Shift Leadership - All housing mentors may be required to act as a Shift Lead during scheduled shifts. Shift Lead responsibilities may include acting as a resource to other Care Attendants. Triaging resident and staff questions and concerns and acting as a point of contact during scheduled shift. Managing communication flow through chain of command. Collaboration with facility nursing to ensure protocols and procedures are properly followed. Offering feedback and education as needed for peers.
  • Other Duties As Assigned - Required to complete assigned online education modules within designated timeframes and additional memory loss training. Will assist in other areas of the building and/or other housing facilities as needed. Ensure company policies, procedures, and guidelines are followed throughout the facility. Education and Experience Required:
  • A Minnesota nursing assistant certification is preferred and may be required for some levels of assisted living.
  • Critical thinking, interpersonal effectiveness and teamwork skills are required.
  • Must complete and pass a state required background study.
  • Able to physically perform the duties required by the position.
  • Required to have the ability to read, write and speak English well enough to communicate with staff, residents and visitors.
