Job Summary: The Care Companion is responsible for engaging members (clients) in service by providing direct care to increase socialization and connect to community and resources in an assigned geographic area. The responsibilities include program outreach, friendly visits (telephonic and in-person), light household chores, personal errands such as grocery shopping and transportation. This position is also responsible for documenting direct care visits of members in service, assuring compliance with program regulations, and managing relationships with multiple stakeholders in the community.This position may also be responsible for the provision of direct client services, including LSS Healthy Transitions and similar services, under the supervision of a certified staff member. Qualifications & Requirements:
- Ability to work well with diverse individuals, families, groups and organizations and demonstrate compassion, patience, flexibility and respect for others.
- Provide regular in-home/telephonic visits to members and provide transportation in the community as needed.
- Have dependable transportation and be willing to transport members in your vehicle.
- Computer skills, bookkeeping, computer spreadsheets and word processing skills required.
- Excellent communication and customer service abilities required.
- Organizational skills, problem-solving and the ability to work both independently and as a member of a team.
- A valid driver's license, a clean driving record and background checks are required.
