Offer Based on Years of Experience What You Need to Know Under the general supervision of the Director of Clinical Programs: Care Transitions, the Care Continuum Liaison responsible for partnering with and liaising between HH, HOS, PCS and PC branches to achieve optimal patient satisfaction & outcomes by supporting/facilitating successful patient transitions between HH, HOS, Palliative Care and PCS in shared areas. This position directly supports the organizational/financial goals of the Care Continuum.
