**Primary Function/General Purpose of Position** _:_
The Primary Care & Care Continuum Strategic Coordinator will support the strategy for the Primary Care & Care Continuum Service Lines (post-acute services including senior care services, home based primary care, palliative medicine, home health, hospice, and others) with a focus on attributed lives through the ACO and other value-based contracts. Working across the continuum, this position will ensure strategies are aligned between services, processes are implemented, and outcomes are monitored to enhance access, operational efficiency, and alignment. The coordinator will manage inter-department and inter-facility relationships that enhance clinical and operational outcomes for defined populations. A data-driven approach will be utilized, including collecting and analyzing data, to drive strategic priorities. The position will collaborate with Richmond market stakeholders, and with shared services such as Population & Community Health.
**Essential Job Functions**
+ _?_ Supports the Primary Care & Care Continuum Councils including scheduling, agendas, minutes and action items to ensure continual progress on objectives and outcomes, in accordance with market strategy for the health and wellness of defined populations, especially serious illness
+ Supports market processes in relation to defined annual goals to ensure access, efficiency, and alignment of Primary Care & Care Continuum Services
+ Creates and maintains mechanisms for ongoing & effective communication between Primary Care & Care Continuum services, including post-acute entities
+ Utilizes performance improvement tools to support Primary Care & Care Continuum initiatives
+ Utilizes and monitors dashboards to ensure success in clinical and operational outcomes for defined populations
+ Develops and maintains relationships with Primary Care & Care Continuum partners and stakeholders
+ Carry out contractual obligations with non-employed and/or post-acute entities as relevant
+ Experienced and knowledgeable regarding primary care and serious illness models and programs that support value based care
