Job Description:
Manages all operation and functions for Care Coordination team which provides care coordination services for the organization, including case management, discharge planning, and psycho-social services for patients treated across the continuum of care. Works in collaboration with the medical and nursing staff to manage length of stay and the care of patients at risk for readmission. Consults with staff, providers, leadership, and the Director of Nursing on patient care matters and interpretation of facility policies and procedures to ensure all Care Coordination and related patient needs are met. Maintains performance improvement activities within the department and participates in continuous quality improvement (CQI) initiatives.Supervises coordination of the department in order to maximize productivity and minimize costs.6. *Completes case management assessments; determines priorities for case management intervention and differential treatment needs of patients and families; documents assessment, plans and interventions based upon departmental standards.7. *Utilizes knowledge of high-risk indicators to identify all forms of family violence; understands impact of different types of chronic illness on the patient and family.8. *Provides supportive counseling to patients and families to enhance coping mechanisms.9. Provides crisis intervention services to patients, families and staff; assists patients, families and staff in problem solving and conflict resolution.10. *Serves as a resource to the inter-disciplinary team and discharge planning.11. *Partners with Care Management Discharge Planning staff to assist in identification and resolution of complex and problematic discharge planning issues.12. Assists patients/families in understanding, accepting and following medical recommendations.13. Facilitates communication with patients/families from a wide variety of developmental and social/cultural backgrounds; conveys an attitude of acceptance, sensitivity and caring.14. *Initiates communication with peers about clinical practice issues.15. Maintains good working relationships with a variety of community service agencies.16. Accountable for the efficient delivery of Care Coordination through effective implementation, evaluation, and utilization of safe, cost-effective, high-quality practices.17. Supervises department operations to achieve quality improvement and patient satisfaction goal/objectives.18. Acts as a liaison with patients, providers, and employees to ensure high-quality patient care.19. Addresses and supports cultural practices as long as the practices do not harm others or interfere with the planned course of medical care.20. Develops, communicates, and implements policies and procedures and monitors compliance.21. Ensures that clear, concise, and current written policies and procedures are available to assist the staff and minimize risk factors.22. Collaborates with senior leadership to use findings from patient satisfaction surveys with input from the staff to establish and implement unit-based customer service initiatives.23. Maintains established staffing plans to ensure reasonable hours and acceptable working conditions exist to provide optimal coverage.24. Provides routine performance feedback and conducts annual performance reviews for staff.25. Collaborates with Human Resources to effectively resolve employee and labor management issues.26. Builds and leads a culture of teamwork and respect throughout the department, engaging effectively with all stakeholders, e.g., physicians, staff, outside contractors, and the people served.27. Provides leadership and mentorship in the development of staff.EDUCATION AND EXPERIENCE:Master's degree in social work (MSW) from a School of Social Work OR master's degree in counseling or related field preferred. Bachelor's degree in social work (BSW) required. Current license as a Licensed Mental Health Practitioner (LMHP) and Licensed Clinical Social Worker (LCSW) by the State of Nebraska required. Prior case management experience desiredOTHER CREDENTIALS / CERTIFICATIONS:Basic Life Support (CPR) certification required.
