The Care Coordinator conducts telephonic counseling to CCM patients and is responsible for documenting and communicating information to and from the providers for effective and coordinated care. This position requires a high frequency of contact with the very high-risk ACO patients and their caregivers for the education of health promotion and self-care skills as well as facilitating compliance with the required quality measures for all ACO patients. The Care Coordinator will also ensure the communication of a patient's treatment plan and general status to all caregivers during all transitions of care. - Diploma/GED Equivalent certification program.
- Registered Certified Medical Assistant (RCMA) or Certified Nursing Assistant (CNA) from the State of Board of Nursing with active certifications with 2 years' care coordination experience
- One to three years' experience in clinical care setting preferred.
- Strong communication skills and computer navigation ability.
- Ability to effectively communicate in English, both verbally and in writing.
- Bilingual Spanish preferred
- Strong work ethic
- Interpersonal skills
- Ability to work cooperatively with members of the healthcare team
- Ability to handle frequent interruptions and adapt to changes in workload and work schedules
- Proficient in computer and keyboarding.
- Self-motivated and able to work well independently.
- Experience and/or strong interest in working with Medicare population.
- Understanding and compliance with HIPAA.
- Ability to document in different electronic health records as well as utilize Microsoft Products.
