Job Summary
Directly supports the YMCAs care coordination process of connecting individuals referred by their health care providers or through self-referral to the YMCAs suite of chronic disease prevention and management programs through engaging in phone- and email-based outreach to these individuals and conducting motivational interviewing conversations with each person. Provides administrative, organizational, and logistical support to the Healthy Living Center of Excellence and assists in quality assurance and oversight of chronic disease prevention programs.
Hiring Range: $22 - $24.20/hour
