Unison Health is hiring experienced individuals to train as full-time Care Coordinators to work with children/youth with behavioral or developmental disability health challenges and their families.You will work closely with children/youths, their families and community partners to link the child/youth to needed care and services.Our service area includes Lucas, Fulton, Henry, Williams, Putnam, Defiance, Paulding, Van Wert and Mercer countiesQualifications needed:
- High School Diploma and 3 years' experience OR Associates or bachelor's degree and two years' experience OR Master's degree and one year experience.
- Experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field providing community-based services to children and youth, their family, or caregivers.
- Expertise in one or more of the following areas: Family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
- Must be proficient in the use of computers and systems.
- Must possess a valid driver's license, reliable transportation, be able to be insured through the agency commercial policy and have personal auto insurance.
- LSW/LPC preferred.
