Responsible for assessing, planning, implementing, monitoring and evaluating options and services required to meet an individual's health need along the healthcare continuum. The focus is to access quality care for patients by collaborating with payers and providers in developing, implementing and evaluating quality, cost-effective treatment options delivered in the most appropriate setting.- Graduate of an approved school of nursing required - BSN required, or currently enrolled in an accredited ASN to BSN or MSN program required- Current licensure as a professional registered nurse in the State of Missouri or licensure eligibility required- Minimum of two years Case Management experience required- Certification in Case Management preferred- Minimum of two years clinical experience required- Proficient computer skills- Self-disciplined, energetic, passionate and innovative- A team player that can follow a system and protocol to achieve a common goal- Highly organized and well-developed oral and written communication skills- Demonstrates sound judgment, decision-making and problem-solving skills- Confidence to communicate and outreach to other community health care organizations and personnel- Knowledge of MO Nurse Practice Act standards and regulations for practice- Maintains BLS validation
