Job Description ECU Health About ECU Health North HospitalECU Health North Hospital is a Joint Commission-accredited, 204-bed acute bed facility serving Halifax, Northampton and Warren counties in the north-central region of North Carolina. Its service area also reaches into southern Virginia. With more than 600 team members, the hospital offers a broad range of medical and surgical services. It features a 24-hour emergency department and offers advanced diagnostics as well as specialty and primary care.Position Summary The Care Coordinator is responsible for the delivery of counseling, case management, discharge planning and related duties to assigned patients. The role provides direct interventions with patients and families jointly with the interdisciplinary treatment team. This includes assessment, treatment planning, individual and group psychotherapy, problem solving, safety, advocacy, community referrals and crisis intervention. It also involves management of discharge planning and treatment team meetings. Care Coordinators work on inpatient psychiatric units with possible cross coverage for other areas.Responsibilities Care CoordinationCompletes thorough psychosocial assessments and conveys information to the treatment team.Coordinates the development/implementation/review of the interdisciplinary treatment plans involving the patient/family, health care team, legal representative, and community supports as needed.Ensures that the mental, behavioral, physical and safety issues of patients are addressed in the treatment team meetings.Provides individual, family, and/or and group psychotherapy.CommunicationResponsible for clinical documentation related to treatment goals, group and individual interactions and discharge planning communication. Responsible for appropriate discharges and placements to other facilities as needed. Identifies and makes referrals to other community resource agencies.Orients new team members to the role when assigned.Represents the department in a positive manner in ECU Health and within the community.Other DutiesAlso may be responsible for determining use of benevolent funds for transportation, medications, and other issuesProvides supervision of assigned students and interns.Wears ECU Health identification badge while on duty.Adheres to ECU Health policy and procedures.Minimum Requirements Master's degree in Social Work, Counseling, Marriage & Family Therapy, and/or Psychology.Licensed in North Carolina at the associate level with an LCSW-A, LCMHC-A, LMFT-A, or LPA.Additional associate license as a Clinical Addiction Specialist (LCAS-A) is preferred (or may be required for some Care Coordinator positions).Supervision in place prior to hire (some exceptions may be made).
