Our client, a leading healthcare organization, is seeking a CARE Coordinator to join their team. As a CARE Coordinator, you will be part of the Client Assessment Referral and Evaluation (CARE)/Pre-Admission Screening and Resident Review (PASRR) Process, collaborating with clients, families, hospitals, Area Agencies on Aging (AAA), Medicaid, level II contractors, and stakeholders regarding communication related to assessors and assessments as well as tracking the assessments. The ideal candidate will have excellent communication skills, attention to detail, and the ability to work collaboratively with a diverse range of stakeholders.Job Title: CARE CoordinatorLocation: Topeka, KansasPay Range: $17.25 per hourWhat's the Job? Coordinate the CARE/PASRR process by collaborating with clients, families, hospitals, AAA, Medicaid, level II contractors, and stakeholders regarding communication related to assessors and assessments as well as tracking the assessments. Assist Care Specialists with processing Special Admissions, CARE Level I Assessments, Level II PASRR Evaluations, and follow-up. Review, evaluate, and follow-up on the 3164 requests from Medicaid. Complete quarterly quality review of the 11 designated AAAs as it relates to data transfer & entry of specified level I's and timeliness of level II referrals. Serve as a point of contact for stakeholders, constituents, and AAAs by developing and maintaining professional relationships with nursing facilities, nursing facilities for mental health, hospitals, AAAs, the Level II contractor, and others as necessary.What's Needed? Excellent communication skills, both written and verbal. Attention to detail and the ability to work collaboratively with a diverse range of stakeholders. Ability to review, evaluate, and follow-up on requests from Medicaid. Experience in coordinating and facilitating stakeholder meetings. Ability to maintain professional relationships with nursing facilities, nursing facilities for mental health, hospitals, AAAs, the Level II contractor, and others as necessary.What's in it for me?
