Join us in making a difference in the lives of our clients and advancing your career!Position Perks & Benefits:29 Days of PTOCompetitive PayEligibility for HRSA Loan Repayment *eligibility requirementsEmployee benefits package - health, dental, vision, retirement, life, & more**Competitive 401(k) Retirement Savings Plan - up to 5% match for Part-Time and Full-Time employeesCompany-paid basic life insuranceEmergency Medical Leave ProgramFlexible Spending Accounts - healthcare and dependent child-careHealth & Wellness ProgramEmployee Assistance Program (EAP)Employee Discount ProgramMileage ReimbursementKey Responsibilities:Review initial assessments and discharge plans to identify patients' needs, including employment, healthcare, education, and social connectedness.Coordinate mental health and substance use disorder outpatient services as needed.Communicate regularly with referral sources and primary care providers, documenting progress, clinical issues, and planning for transitions.Request and review records from previous treatment providers and primary care physicians.Participate in interdisciplinary team meetings to ensure continuity of care.Provide referrals for services not available within the treatment program.Assist with scheduling treatment sessions and coordinating transportation as needed.Develop and update individualized treatment plans based on patients' evolving needs.Provide crisis intervention and support for patients as necessary.Document all services provided according to regulatory standards and maintain accurate records.Facilitate transitional planning and advocate for patients' needs within the community.Assist patients and families in accessing community resources and support services.Maintain weekly direct service expectations and adhere to agency policies and standards.Education and/or Experience Qualifications:Bachelor's Degree in social work, psychology, human development, criminal justice, counseling, nursing, education, or a related human services field is preferred.Additional Qualifications:Knowledge of case management and rehabilitation methods related to mental health and substance use disordersFamiliarity with challenges faced by individuals with mental illness and substance use disorders and community resources availableUnderstanding of the legal requirements and processes in treating mental health and substance use disordersStrong verbal and written communication skills for producing clear and concise reportsAbility to evaluate program services and offer constructive feedbackSupervisory Requirements:NoneBrightli is on a Mission:A mission to improve client care, reduce the financial burden of community mental health centers by sharing resources, a mission to have a larger voice in advocacy to increase access to mental health and substance use care in our communities, and a mission to evolve the behavioral health industry to better meet the needs of our clients.As a behavioral and community mental health provider, we value diversity, equity, and inclusion in our workforce and encourage applications from individuals from diverse backgrounds and experiences.
