The Care Coordinator is responsible for ensuring the care of program participants. Assist in developing and working actionable life plans. Coordinate resources and referrals for a caseload of program participants. Continually assess and create a safe environment that is conducive to change and thatassures individual commitment to personal change. Ensure the development of individualized plans for each participant; facilitate ongoing plan changes that support individual progress, needs, and challenges. Lead, guide, encourage, and support volunteers who will work closely with program participants as mentors, teachers, community resource specialists, post-graduation support, and prayer warriors. Create and sustain a team environment, where God is the center and interdependence is valued. The care coordination will focus on LaunchPoint and LaunchPoint Phase 2 students. Duties:
- Ensure the consistent application of program standards and guidelines, curriculum, and discipleship models.
- Develop relationships with participants that promote and model a well-balanced Christian life.
- Ensure the development of individualized plans for each program participant.
- Meet with participants as necessary to ensure maximum benefit from KARM programs and services.
- Make appropriate internal and external referrals for services.
- Ensure consistency in application of KARM safety guidelines, policies, and procedures that impact participant activities and decisions.
- Work with volunteer leaders in key support areas (mentoring, aftercare, prayer, etc.)
- Ensure the maintenance of accurate, up to date, file records for participants, utilizing HMIS and other applications and tools provided.
- Exhibit caring Christian lifestyle to residents, staff, volunteers, and donors
- Work cooperatively with ministry staff and departments to ensure an effective and efficient residential environment.
- Coordinate work processes with other departments as needed.
- Help manage and maintain the classroom for a quality learning environment, aesthetically pleasing, and functional.
- Assist with assigning and managing work assignments and laundry priviledges for LP and LP2 students.
- Perform other essential duties as assigned.
Requirements:
- BS in related field (psychology, social work, counseling, ministry) or equivalent education and experience
- Interest in working with the homeless and/or those with substance abuse and/or mental health challenges
- Ability to clearly explain and present the Christian plan of salvation and lead a person through the process
- Demonstrated ability to establish a feeling of trust, safety, consistency, and hope
- Demonstrated ability to work cooperatively with peers and management toward the accomplishment of the ministry purpose
- Experience with and skilled in leading and coaching others
- Ability to set and monitor goals for and with individuals
- Demonstrated skills in individual and group counseling
- Ability to maintain organized records
- Ability to effectively adapt to change
- Good organizational and administrative skills
- Ability to manage a busy work calendar independently and with self discipline
- Ability to build personal relationships and maintain appropriate boundaries with residents
Working Conditions:
- Potential high-risk environment at KARM maintained property
- Working daily with individuals having medical, mental health, and/or addictive behaviors
- Some local travel may be required for various purposes
- Occasional required attendance at KARM special events, sometimes during weekend or evening hours
