We serve upward of 5,000 individual children per year, as well as their siblings, guardians, and family members (totaling more than 15,000 individuals). and Compensation for Care Coordinator:* Starting hourly rate $17.10* Additional pay for bilingual skills* Competitive compensation* Comprehensive Benefits Package* Flexible Spending Plan for medical and dependent coverage* Annual Professional Development stipend* Unparalleled mentoring and support* Plus many more!!Job Summary for Care Coordinator:* Utilizes a short-term case management model to provide resources, brokering and education for a minimum of three days following the crisis assessment.* Follows up and links the client with specified services and resources as identified by the therapist or determined beneficial by the Care Coordinator* Immediately alerts the direct supervisor of any unusual problems, anomalies or potential crisis that arises with clients.* Complies with agency policies and procedures as well as, funding and regulatory guidelines and standards.* Ensures that routine paperwork is completed accurately within the required timeframe.RequirementsEducation and Experience Requirements for Care Coordinator:* Bachelor's degree in a social services field is required.* A minimum of one year of experience working with adults in the behavioral health field.* Good computer skills and ability to adapt to electronic health record changes.* CGC is a Drug Free Workplace *ADA Disclaimer: In developing this job description, care was taken to include all competencies needed to successfully perform in this position.
