- Provide varying types of case management and/or rehabilitative services in various setting to individual, families and groups.
- Provide crisis intervention services, as assigned.
If assigned Children's caseload, the following applies:
- Schedule, coordinate and facilitate wraparound/family teams.
- Complete Strengths Needs Cultural Discovery (SNCD) assessments, Care Plans and Updates, Functional Assessments/Crisis Plans/Safety Plans.
- Complete flex-fund requests when necessary.
- Monitor and evaluate services provided to the families. Contact families as needed to monitor progress and inquire about new needs.
- Facilitate or co-facilitate Family Team meetings and may type the agenda.
- Schedule and coordinate Family Team meetings and ensure both formal and informal supports are included and part of the team.
- Contact referred families and the referral source for screening.
- Provide the appropriate staff personnel with new family team lists and approved referrals.
- The Care Coordinator function will be initiated through a non-diagnostic interview with the client and his/her family to assess service needs. The Care Coordinator will identify needed services by completing a strengths assessment and develop a care plan. The Care Coordinator will recognize that the individual juvenile client and his/her family has different barriers to social, economic, and employment opportunities and has varied levels of need to overcome these barriers. The goals for each juvenile client as identified in the family team will be tailored to reflect a juvenile clients particular needs in order to realize his/her full potential.
- The coordination of services between agencies will be an ongoing task for the Care Coordinator. The Care Coordinator is expected to develop cooperative working relationships with all agencies serving juveniles from which a client might be expected to receive direct or indirect services as well as establishing and maintaining a therapeutic relationship with those juvenile clients and their families who are in need of support. In addition, the Care Coordinator is expected to possess skills necessary to provide immediate crisis intervention on a twenty-four (24) hour basis.
- The Care Coordinator will need to maintain close communication with the Integrated Team Manager and other clinical and administrative staff in order to assure the highest quality of services are delivered to the juvenile client and his/her family.
- Provides targeted support services, a system of care that is child centered with the needs of the child and family dictating the types and mix of services provided and to assist in preventing an out-of-home or out-of-school placement.
QualificationsBachelors degree in Psychology, Social Work, Sociology or Criminal Justice, or related field. Minimum of one-year work experience in the mental health field or related social service field, preferred.Current certification as a Behavioral Health Case Manager II.
