OverviewHome Helpers Home Care of South Boston is seeking a dedicated Care Coordinator/Lead Caregiver to join our team. The Care Coordinator/Lead Caregiver will play a crucial role in supporting the coordination of client care, caregivers and ensuring smooth operations.BE B.O.L.D.Bring your passioneverydayOwn your resultsLove what you doDriven by excellence Duties Cover open shifts for new clients and transition to caregiver with updated care plan; Develop Plan of Care from the required software; Ensure all clients have their appropriate caregiver at their scheduled time; Assist with hiring and orientations; Maintain updated client information in the required software - authorizations, address, phone numbers, contacts, etc.; Enter new clients and match with caregivers in the required software; Prepare caregiver/client schedules in the required software; Find replacement caregivers when regular staff is unavailable (vacations, illness, etc.) according to guidelines; Create documented events and print end of month report by caregiver;Maintain Client Files - filing, updated info, etc. according to Guidelines;Follow up on Incident Reports Within 24 hours, following Guidelines;Ensure Client Notebook/Folder with the Minimum of Plan of Care and Schedule is in each client's file;Complete 90-day and annual caregiver performance appraisals; Schedule meeting with caregiver to review performance; Attend weekly administrative and staff meetings and trainings as requested; Attend health fairs, job fairs, etc. as requested; Assist with organizing meetings, in-services and gatherings as needed; Call client after first shift of new caregiver; Call new caregiver after first shift worked and end of first week; Report pertinent information as needed. Skills 1. Must have a high school diploma, college degree or be an LPN licensed in the state in which services will be provided; 2. Must possess effective communications skills, be flexible and have the ability to guide others; 4. Customer service experience will be helpful in dealing with clients and caregivers; 5. Must be able to work in a face paced environment, handle stress, and be flexible; 6. Maintain a professional appearance and demeanor; 7. Good work ethic; 8. Be able to uphold company code of ethics which includes honesty and integrity; 9. Ability to learn company software; 10. Supervisory experience including employee counseling; 11. Ability to communicate well with people from all walks of life; 12. Ability to speak in public. The owner is very invested in her business and truly cares about her staff and Clients. The office staff is very professional and does a great job managing. As a caregiver you can have as many or as little hours as you wish. Schedules do change but that's only when the clients need changed. I also had an updated care plan for every client and I was able to assist with making sure that my clients had the best care as my shift ended. Supervisors are not at every clients house all the time but they are very involved and always reachable.#J-18808-Ljbffr
