Do you want to work in a healthy, rewarding, and fulfilling environment?COME JOIN OUR TEAM!Located in Colorado Springs, COComprehensive benefits, PTO, and 401(k) available to full-time employees starting on the 1st day of the month following 30 days of hire.JOB DESCRIPTIONJob Title: Care Coordinator Manager FLSA Status: Full Time, exempt, RegularWork Schedule: 40 hour per week (normally)Department: Health and Wellness (#100) Reports to: Director of Health and WellnessGENERAL PURPOSE OF JOB The Care Coordinator Manager at Silver Key is dedicated to serving vulnerable seniors in the Pikes Peak region, including those facing psychiatric illnesses, substance abuse, or other personal and social care challenges. The Care Coordinator Manager functions as both a clinical resource and administrative leader, providing critical oversight in developing and executing care plans that promote member independence and dignity through whole-person care. This position ensures that high-quality services are delivered by guiding and supporting a team of care professionals in the assessment, stabilization, and empowerment of members.ESSENTIAL DUTIES AND RESPONSIBILITIESClinical and Administrative Coordination
  • Team Leadership: Manage and support a team of providers including case managers and resource navigators, ensuring they provide effective, compassionate care aligned with Silver Key's mission.
  • Member Assessment and Treatment Planning: Oversee comprehensive biopsychosocial assessments, addressing the member's physical, psychological, and social needs. Develop and review individualized treatment plans that incorporate input from multidisciplinary teams.
  • Crisis Stabilization: Ensure that members in acute need receive timely crisis intervention, and empower members to develop coping skills and resiliency through strategic clinical approaches.
  • Clinical Oversight: Monitor the implementation of evidence-based practices, including Trauma-Informed Care, Motivational Interviewing, and strengths-based evaluation. Act as a clinical resource to guide team members in addressing complex member cases.
  • Resource Coordination: Facilitate connections with external resources, including primary care, mental health services, housing assistance, and financial support. Act as a primary liaison between Silver Key and other community providers.
Administrative Responsibilities
  • Staff Development: Conduct ongoing training for team members on best practices in clinical care, case management, and member relations. Coordinate with the Director of Health and Wellness to provide support in organizing licensure supervision hours, fostering professional growth and development. Partner closely to ensure structured assistance for team members pursuing additional licensure and certification opportunities.
  • Volunteer Engagement: Partner with Silver Key's Volunteer Engagement team to foster a positive and supportive environment for volunteers working alongside care staff.
  • Compliance and Documentation: Ensure all clinical documentation meets state and organizational standards. Monitor compliance with relevant policies and procedures, conducting regular reviews and audits.
  • Policy Implementation: Work closely with the Director of Health and Wellness to establish and maintain policies for member care, team protocols, and risk management practices.
  • Grant and Program Expectations: Meet assigned grant deliverables, objectives, and reporting requirements. Track and document activities to demonstrate compliance with grant expectations.
  • Member Transition and Follow-up: Oversee member transitions through discharge planning and follow-up assessments, aiming to maximize member self-sufficiency and reduce hospitalizations and emergency service use.
Program Development, Metrics, and Reporting
  • Program Oversight: Develop, implement, and monitor key clinical programs, ensuring goals are aligned with Silver Key's mission and community needs.
  • Data Management and Reporting: Collect, analyze, and report data on program effectiveness, member outcomes, and operational efficiency. Produce quarterly and annual performance reports for organizational leadership.
  • Grant Metrics and Productivity Targets: Achieve specified metrics and productivity targets as outlined by grant guidelines and organizational goals, adapting strategies to meet and exceed expected outcomes.
  • Quality Assurance: Drive continuous quality improvement initiatives to enhance care delivery and member satisfaction.
  • Other duties as required
SUPERVISORY RESPONSIBILITIESThe Care Coordinator Manager supervises a team of case managers, and support staff, ensuring high standards in member service delivery and compliance with organizational goals. Supervisory duties also include managing employee time-off requests and assisting with payroll preparation tasks as assigned. Ensure accurate tracking and timely submission of attendance and payroll-related information.VOLUNTEER ENGAGEMENT RESONSIBILITIESSilver Key is a volunteer centric organization.   • Master's degree in psychology, social work, human services, or a related field with state-level candidate registration or licensure.
DESIRABLE, PREFERRED, OR SPECIAL QUALIFICATIONS
  • Preferred experience in clinical case management, with knowledge of Medicare, Medicaid, federal, and state resources.
TECHNOLOGY PROFICIENCY This position requires good proficiency in the use of human resources systems, computer software and applications, office equipment, and professional telephone use. The ability to type quickly with few to no errors is also very important. This position will often require multi-tasking in the use of computer, phone, and office equipment while interacting with clients. Candidates for this position may be required to take and pass technology proficiency assessments prior to employment.LANGUAGE SKILLSDemonstrated strong verbal and written communication skills, analytical skills, statistical and mathematical abilities, and interpersonal skills. Ability to read and interpret documents such as safety rules, operating maintenance instructions and procedure manuals. Ability to write complex reports and distill information for many audiences. Proficiency in non-English languages is highly valued.MATHEMATICAL SKILLSAbility to add, subtract, multiply and divide in all units of measure using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.REASONING ABILITYAbility to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.CERTIFICATES, LICENSES, REGISTRATIONS
  • Must possess and maintain a Colorado Driver's License
PHYSICAL DEMANDSThis position requires standing, walking and sitting for periods of time. Utilization of hands and fingers to operate office computer keyboard and equipment required. Must be able to reach with hands and arms, talk, hear, and stoop, kneel or crouch if needed. Must be able to lift up to 25 pounds as needed.
