Care Coordinator Supervisor who will be the Care Coordinator's Team Leader. The Care Coordinator Supervisor will guide, support and evaluate the Care Coordinator's individual efforts by focusing on ongoing clinical and compliance issues, staff development, concurrent clinical review and assistance to Care Coordinator with both daily caseload and crisis management.THE POSITIONCareStar is looking for motivated individuals who:
- Possesses an active license as a Licensed Social Worker, Licensed Independent Social Worker or Registered Nurse; AND/OR Bachelor's Degree in Nursing, Social Services or related field with a minimum of 2 years of full-time direct service experience. Experience must be relevant to the program.
- Will complete all training curriculum and professional development requirements as specified by the assigned program.
- Possesses integrity, self-discipline, with the ability to work independently within guidelines.
- Possesses knowledge of Medicare, Medicaid, insurance and waiver programs and community resources.
- Has great problem-solving skills.
- Possesses effective verbal and written communication skills with attention-to-detail in assessment and in documentation.
- Has familiarity with the suite of Microsoft Office programs.
- Possesses a valid driver's license and car insurance as required by State law.
- Adheres to the CareStar Rule in performance of job responsibilities.
- Understands and complies with CareStar Policies and Procedures.
- Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
- Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
- Directly supervises, manages and oversees staff, including hiring, firing and providing performance evaluations, including and individual development goals and objectives.
- Educates, counsels, evaluates, and manages performance of the clinical team.
- Provides care coordination services to consumers as necessary to assist the clinical team.
- Maintains/updates their knowledge of Medicare, Medicaid, insurance and program specific administered waivers.
