This role serves as an expert resource for complex patient situations and consults with other care team members regarding psychosocial and resource needs. In collaboration with the patient and physician, the SW Sr will assess, coordinate, and implement a timely and safe discharge plan to the next appropriate level of care. The role integrates and coordinates the patient's transitional care plan into their individualized discharge plans based on needs and resources available. Core Responsibilities and Essential Functions: - Complex Disposition Planning: Implement discharge planning and provide resource information for complex patients. Identify and document barriers for timely disposition. Understand eligibility processes and criteria for various resources to assist in planning safe transitions for discharge. - Assessment: Initiate assessment of patients' psychosocial risk factors and resource availability for discharge. Collaborate with financial counselors and care team members to ensure adherence to insurance and coverage requirements. - Documentation: Complete initial psychosocial and functional assessments and ensure comprehensive documentation in medical records. Track avoidable days and report trends leading to undesired outcomes. - Precepting/Mentoring: Mentor new hires and less-senior employees in addressing challenging situations. Serve as a mentor for student interns. - Professional Development and Initiative: Complete all professional competency assessments and participate in the development of protocols and performance improvement activities. Performs other duties as assigned and complies with all organizational policies, standards of work, and code of conduct.
