Provide case management services to children in foster care, natural parents, and foster families. Coordinate services and treatment with other program staff, schools, DCYF, and juvenile court. Serves as a member of an organization that welcomes, engages, and serves individuals with complex wellness needsResponsibilities:Provide case management services to children in specialized foster care designed to insure appropriate care and progress towards treatment goals.Provide support, consultation, and on-going training to foster parents, regarding children in placement.Coordinate services with DCYF caseworkers, juvenile court, school personnel, and with other team members and system representation. Participate in team staffing. Arrange staffing on a regular basis and emergency staffing as needed.Arrange pre-placement visits, placement, and parent contacts.Facilitate CFT meetings monthly.Provide skills training, ILS activities, or other pro-social skills to youth.Encourage each CFT member to identify their priority concerns, work proactively to minimize areas of potential conflict, and acknowledge the mandates of others involved in child-serving systems.Utilize consensus-building techniques to meet the needs of the youth and family.Engage all CFT members and identify their needs for meeting agency mandates. The Care Coordinator identifies the strengths and needs of youth and family, provides CFT members with an overview of CFT practice, and clarifies their role and responsibilities as a team member in this process.Work with the Youth Partner and/or Family Partner to identify family support, peer support or other system and community resources that can assist the youth and family.Share in the on-call status rotation.Maintain individual consumer records in a manner consistent with expectations.Share in the provision of crisis intervention services to children and foster families.Support and coordinate therapeutic services with foster parents and natural parents.Ensure that ethical and professional standards of practice are maintained.In addition to the above, perform other duties as assigned.Qualifications:Bachelor's degree. One year experience in a child serving system.
