We offer medical detox as well as residential addictions treatment for active duty and retired military service members in our North Stonington location. Our clinic in Groton, CT, offers partial hospitalization and intensive outpatient treatment, and is conveniently located across the street from The Inn at Trails Corner, a new 180 bed dormitory-style sober community for adult men and women who attend our outpatient day treatment programs. The Inn offers overnight accommodations, full-meal service, indoor/outdoor recreation options, a workout room, and computer center for all clients. We accept private insurance, state-funded plans and self-pay arrangements. Job Summary: The primary role of the Care Coordinator is to coordinate admission and transition to and from care of each client based on a person-centered process which is driven by an individualized treatment plan that functions to maintain the individual's well-being and prioritize treatment goals and objectives for a safe transition from care. The Care Coordinators functions as an expert in the community and recovery resources and participates as an independent member of the multi-disciplinary treatment team to ensure coordination of care from admission to discharge. The Care Coordinator uses that knowledge to assist clients in accessing the network of resources that will help maintain their recovery to promote further growth to maximize functional independence. Job Duties and Responsibilities:
  • Assures that the functions that comprise case management and/or care coordination - assessment, planning, linkage, monitoring, and advocacy - must always be adapted to fit the particular needs of the client and compatible with the military structure.
  • Ensures patients are oriented to program at time of admission with the support of Residential Aids, Clinical Team and other members of Starlight.
  • Works collaboratively with Director of Clinical Services, other members of the team, clients, and referral sources/community partners to incorporate client needs and preferences and the recommendations of the multidisciplinary treatment team into the admission & discharge planning process.
  • Assures that care coordination services assess the client's readiness for change, identify client strengths or problem areas that may affect the processes of treatment and recovery, and engage the client in the development of a collaborative treatment and discharge plan.
  • Assures that care coordination services function as an advocate for the client.
  • Attends and participates in weekly Treatment Team Meetings to ensure patients aftercare recommendations are monitored and maintained prior to discharge.
  • Is responsible for the completion and monitoring of Fidelity Program Discharge Documentation (checklists, aftercare plans, client safety plans and other required documentation)
  • Understands the insurance, commercial/private insurance, VA insurance and health maintenance options available and works to help clients access those benefits.

Qualifications Bachelor's Degree preferred in related field preferred. Knowledge of case management processes is preferred. Prior intake, assessment, admission and crisis intervention experience with a psychiatric mental health and/or chemical dependency population, preferably in a day treatment, residential, or inpatient setting a plus. Knowledge of insurance plans and mental health and chemical dependency treatment coverage preferred. Experience with computer data entry preferred. Prefer knowledge of crisis intervention and risk assessment, psychiatric diagnoses symptomology, chemical dependency diagnoses and symptomology, age specific growth and development and crisis and behavior management.
