Care Delivery Manager RN HealthCare Support is actively seeking a Care Delivery Manager RN to fill an opening in Ridgewood, NY.Daily Responsibilities Care Delivery Manager RN:
- Oversee Personalized Care Plans: Develop and execute care plans tailored to participants, ensuring improved satisfaction and outcomes.
- Facilitate Meetings and Documentation: Manage IDT meetings, Service Delivery Requests, and necessary attestations; support Virtual IDT and ensure timely follow-ups.
- On-Call Management: Handle responsibilities during non-operational hours, including documentation, grievance initiation, and participant/caregiver follow-ups.
- Monitor and Review Care: Run clinical delivery reports, review charts for quality, and coordinate long-term care services, adjusting care plans as needed.
- Staff and Regulatory Oversight: Manage staffing, participate in HR functions, ensure compliance with regulations, evaluate staff performance, and resolve grievances.
- Shift: 8:30am - 5pm (1 hour Break)
Required Qualifications for Care Delivery Manager RN: Education:
- Degree from an NLN accredited nursing school (RN, BSN required or at least five (5+) years' experience in health care setting).
- Successful completion of an accredited Nursing Program is required.
- A Baccalaureate degree is highly desired.
- Completed NYSED-approved infection control coursework.
- A minimum of 8.0 education contact hours may include contact hours of mandatory training requirements such as infection control coursework.
- Case Management Certification recommended within two (2) years of the employment.
Experience:
- Minimum of two (2+) years of administrative experience in a management capacity in a certified home health agency (CHHA), long-term home health care (LHCSA), acute care, medical-surgical, and/or critical care, nursing home experience, diagnostic & treatment clinic preferred.
- Customer Service experience is required.
- Managed long-term care insurance experience beneficial.
- Minimum of one (1+) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
- Supervisory experience is required.
