A health services network in New York City is currently seeking an experienced, licensed Registered Nurse (RN) to join their team in the Bronx as a Care Delivery Manager. Responsibilities: The Care Delivery Manager (RN) will:
- Conduct UAS assessments on patients to determine their healthcare needs
- Develop care plans based on UAS assessments
- Continuously monitor and evaluate the patient's progress and adjust the care plan as necessary
- Educate patients and their families about their care plans, including medication management, self-care, and other relevant aspects of their healthcare
- Participate in quality improvement initiatives to enhance the overall care delivery process and patient outcomes
- Advocate for patients and ensure their needs and preferences are considered when developing and revising care plans
- Compile and report data related to patient assessments, care plans, and outcomes as required by regulatory agencies or the organization
- Stay up-to-date with UAS assessment guidelines, nursing best practices, and industry developments, and provide training and education to staff as needed
Qualifications
- 3+ years of relevant experience
- Bachelor's Degree in Nursing
- NYS Registered Nurse (RN) license
- Experience working with the Elderly population
- Experience in a Supervisory role
- Solid assessment and documentation skills
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
Desired Skills:
- 2+ years of Administrative experience in a Management capacity within a Certified Home Health Agency (CHHA), Long-term Home Healthcare (LHCSA), Acute Care, Medical-Surgical, and/or Critical Care setting
- Nursing Home experience
- Experience with Diagnostic & Treatment clinic
- Managed Long-term Care Insurance experience
