Our passion for positively impacting the lives of residents, administrators, and healthcare professionals drives our commitment to continuously innovate and redefine senior care across the country.Position Details:Assigns and schedules patients to be seen for MedElite's care management program.Responsibilities:Program Development: Identify areas for improvement and implement strategies to enhance program effectiveness. Conduct regular assessments to ensure alignment with industry best practices. Patient Engagement: Coordinate patient enrollment in care management programs. Care Coordinations Schedule patients to care coordinators, ensuring an equitable distribution of patients daily, weekly, and monthly among the team members. Collaborate with healthcare providers, including physicians, nurses, and care teams, to ensure seamless care coordination for patients enrolled in the care management program. Documentation and Reporting: Generate reports and analyze data related to program outcomes, patient satisfaction, and adherence to quality standards Compliance and Quality Assurance: Ensure that the care management complies with all relevant regulations and industry standards.Implement quality assurance measures to guarantee the delivery of high-quality services to patients. Identify areas for program improvement and develop strategies to enhance the quality and effectiveness of the care management program. Participate in quality improvement initiatives and monitor program performance metrics.
