Job Duties include:
Maintain continuous care management services of a complex caseload over extended periods of time and various levels of care as well as understanding eligibility determination for program.
Will provide self-care management education and training, advocacy, and information dissemination
Assist client with understanding their care management plan and following through with provider recommendations and medication regimen as prescribed by provider
Educate and assist client with self-management plans to manage their chronic health care conditions; diabetes, hypertension, HIV/Aids
Links and advocates client to essential resources in the community.
Participate in regular multi-disciplinary staff meetings.
Develop relationship and communication with all treatment providers and family members to ensure continuity of care.
Meet regularly with supervisor as a means of enhancing professional growth, reviewing and processing the provision of care management services and dealing with administrative issues.
Documents all service contacts on a timely basis and maintains case records in accordance with agency standards and requirements
Assist in the coordination of care including tracking clients, prompting for appts. and referrals, attend appts. with clients (if requested), provide linkage to treatment providers including auditory, vision, dental care and social support as needed.
Qualifications:
Bachelors Degree (psychology, social work, or Behavioral Health related field)
1-2 years of case management or related experience
Valid Drivers License and auto insurance
Schedule: Monday - Friday: 8:30am - 5pm
Pay range: $25-28/hr
