Join Eliot and get to work alongside our talented and dedicated staff specializing in housing navigation, recovery coaching, case management, peer supports and other specialized stabilization services.Responsibilities:Provide side by side interventions to individuals that focus on problem solving, skills training, modeling behavior, optimism and encouragementProvide interventions in the office, virtually, in the community and in client's residences based on client preference, client need, and at the clinical direction of the TeamEnroll eligible individuals in care management, including BHCP and Health HomeCollaborate with Care Transitions Team members to triage hospital admissions data and formulate assertive follow-up strategies to contact hospital treatment teams and enrollees during inpatient admissionsAssist the team and individuals in providing on-going assessment of symptoms and responses to staff interventionsParticipate in the development of treatment plans for each individual in which you are a member of their treatment team.Ensure that treatment goals are culturally competent, strength based, realistic, attainable and promote recovery. Ensure that each individual is actively involved in their treatment planning process and their preferences are embracedEnsure that documentation is written is measurable, observable, objective and utilizes clinical termsCoordinate and ensure timely face to face contact with enrollees in the community following inpatient discharge within timeframes dictated by MassHealth contractQualifications:Must have bachelor's degree or master's degree in psychology, social work or related field and three years experienceMust have a strong commitment to recovery oriented servicesMust have skills to establish supportive relationships with persons with severe and persistent mental illnesses, kids, families and respect for their preferences for treatmentMust have ability to communicate clearly and effectively with individuals in rehabilitation and treatmentSalary:$50,000Hours: Monday-Friday, 9am-5pm with remote/hybrid flexibility(3 days on-site, 2 days remote)Bilingual Differential: $3,000Eliot is committed to identifying and dismantling barriers that prevent people with marginalized racial identities from actualizing their full employment potential, assuming leadership roles, or from fully engaging at all levels in the workplace.
