Position Overview:We are seeking a compassionate and experienced homecare manager to join our team at Assisting Hands Serving Carlsbad and Oceanside. The Care Manager will work directly with our owners, playing a pivotal role in overseeing the delivery of care solutions and services to our clients. The ideal candidate will be responsible for coordinating care plans, assessing client needs, supervising caregivers, and serving as a liaison between clients, caregivers, and other healthcare professionals. Responsibilities:Conduct comprehensive assessments of client needs and develop individualized care plans in collaboration with clients, their families, and other healthcare professionals.Coordinate and schedule care services based on client preferences, availability of caregivers, and specific care requirements.Recruit, train, supervise, and evaluate caregivers to ensure they deliver exceptional care in accordance with company policies, procedures, and standards.Conduct regular home visits to assess client satisfaction, monitor the effectiveness of care plans, and address any concerns or issues that may arise.Serve as a point of contact for clients and their families, providing support, guidance, and advocacy as needed.Collaborate with healthcare providers, including physicians, nurses, therapists, and social workers, to coordinate and integrate care services effectively.Maintain accurate and up-to-date documentation of client assessments, care plans, and service delivery in accordance with regulatory requirements and company standards.Qualifications:Minimum of 5 years experience as a care manager, LVN or RN.Skilled problem solver with a can-do attitude. Strong interpersonal, communication, and leadership skills with the ability to collaborate effectively with clients, caregivers, and office team members.Proficiency in computer skills, including Microsoft Office applications. Experience with care management and scheduling software preferred.Valid driver's license and reliable transportation.Compassionate, empathetic, and client-centered approach to care management.
